Cochlear Implant Testing

Auditec customers often tell us they need tools to test cochlear implant users. We sell three recordings specifically designed for cochlear implant users as well as many other tests that could be used with cochlear implant patients. The three tools are 1) Lexical Neighborhood Test/Multi-syllabic Lexical Neighborhood Test (LNT/MLNT), 2) BKB-SIN, and 3) Minimal Auditory Capabilities (MAC). All three of these include recordings and instructions so that customers can test cochlear implant patients with confidence.

Since children with cochlear implants generally perform poorly on the PBK word list, it is not a recommended recording for cochlear implant patients.  Instead, we recommend the Lexical Neighborhood Test/ Multi-syllabic Lexical Neighborhood Test (LNT/MLNT). It was specifically created to assess open-set speech performance in cochlear implant children.

Since cochlear implant users report speech recognition is more difficult in noise, audiologists need a tool to test cochlear implant patients with noise. The BKB-SIN (Speech In Noise) uses sentences in four-talker noise at varying signal-to-noise ratios. This test has also been used to determine cochlear implant candidacy.

If traditional speech materials are too difficult to evaluate hearing ability in an adult cochlear implant patient, we recommend the Minimal Auditory Capabilities (MAC) Battery. It includes gross sound identification, inflection detection, contrast detection, accent discrimination, and word identification with subtests such as question/statement, vowel recognition, spondee recognition, noise/voice, accent, SPIN High-context sentence,  familiar sound recognition, monosyllabic word discrimination, and more.

The LNT/MLNT, BKB-SIN, and MAC Battery are three different tools audiologist can use for cochlear implant patients. However, there are other recording selections in Auditec’s catalog that may be suitable depending for your your needs such as noise recordings, environmental sounds, or auditory training. BKB-SIN©

The BKB-SIN© test is a speech in noise test for children. It was developed for children (ages 5 and up), cochlear implant patients, and adults for whom the QuickSIN© test is too difficult. Other uses include: demonstrating the benefits of amplification, predicting performance with hearing aids in loud, noisy environments, assessing directional microphone performance, estimating children’s performance for soft speech, and screening for auditory processing disorders in children.

The BKB-SIN© uses 18 equivalent list pairs for estimating SNR loss. It features Bramford-Kowal-Bench sentences in four talker babble, uses easier sentences than the QuickSIN©, and has norms for children, adults, and cochlear implant users. CD 1 (Standard BKB-SIN) has the target talker and background babble recorded on the same channel of the CD, at pre-recorded signal-to-noise ratios.  CD 2 (Split Track BKB-SIN) has the target talker and background babble recorded on separate channels of the CD.

The BKB-SIN© includes a) audio recording (CD1 and CD2), and b) manual and score forms. The 27-page manual is comprised of administration, interpretation, and development. Normative data for ages 5 and up is included.

NU-6 Mixed with Multitalker Noise at +9dB S/N©

This recording consists of the NU-6 Form C word list mixed with Multitalker Noise at a signal to noise ratio (SNR) of +9 dB. There are four lists of 50 words each (200 words total). Children may be unfamiliar with the vocabulary of this adult word list. Therefore, it may be necessary to use a speech in noise test designed for children such as the BKB-SIN (sentences in noise).

Speech In Noise©

The Speech in Noise© (SIN)¹ test was developed to help determine how much a given hearing aid or pair of hearing aids helps an individual understand speech in a background of noise, compared to listening unaided or with other hearing aids. The test consists of a series of female-talker recordings of the IEEE (modified Harvard) sentences as the target speech, and four-talker babble (three women and one man) as the noise source. Five sentences are recorded at each of four signal-to-noise (S/N) ratios: 15 dB, 10 dB, 5 dB and 0 dB S/N, all at the 0 dB VU meter calibration level, followed by a similar series recorded at a signal level 30 dB lower than the calibration level (-30 dB re 0 VU).

Therefore, there are 20 sentences at each level for a total of 40 sentences (or 4 IEEE lists), all of which constitutes a block. There are nine blocks of the SIN test, so that nine comparisons can be made without repeating sentences. Each listening condition takes approximately twelve minutes to administer.
