Cochlear Implant Testing

Auditec customers often tell us they need tools to test cochlear implant users. We sell three recordings specifically designed for cochlear implant users as well as many other tests that could be used with cochlear implant patients. The three tools are 1) Lexical Neighborhood Test/Multi-syllabic Lexical Neighborhood Test (LNT/MLNT), 2) BKB-SIN, and 3) Minimal Auditory Capabilities (MAC). All three of these include recordings and instructions so that customers can test cochlear implant patients with confidence.

Since children with cochlear implants generally perform poorly on the PBK word list, it is not a recommended recording for cochlear implant patients.  Instead, we recommend the Lexical Neighborhood Test/ Multi-syllabic Lexical Neighborhood Test (LNT/MLNT). It was specifically created to assess open-set speech performance in cochlear implant children.

Since cochlear implant users report speech recognition is more difficult in noise, audiologists need a tool to test cochlear implant patients with noise. The BKB-SIN (Speech In Noise) uses sentences in four-talker noise at varying signal-to-noise ratios. This test has also been used to determine cochlear implant candidacy.

If traditional speech materials are too difficult to evaluate hearing ability in an adult cochlear implant patient, we recommend the Minimal Auditory Capabilities (MAC) Battery. It includes gross sound identification, inflection detection, contrast detection, accent discrimination, and word identification with subtests such as question/statement, vowel recognition, spondee recognition, noise/voice, accent, SPIN High-context sentence,  familiar sound recognition, monosyllabic word discrimination, and more.

The LNT/MLNT, BKB-SIN, and MAC Battery are three different tools audiologist can use for cochlear implant patients. However, there are other recording selections in Auditec’s catalog that may be suitable depending for your your needs such as noise recordings, environmental sounds, or auditory training. Next, Auditec sold auditory test materials on cassette tapes. Believe it or not, we still get requests for this discontinued format. At one point in time, Auditec even made its recordings available on digital audio tapes (DATs).

MP3s are a convenient next step for professionals who want to use portable equipment that does not take up a lot of space like iPods, iPads, and MP3 players. (This article was edited on March 23, 2017 to include recent research by Jennifer M. Brace and Robert W. Keith.) Recent research has concluded normal listeners tested using the mp3 audio files of speech discrimination tests produce the same valid results as when tested using the wav audio files on compact discs.

NTID Speech-Reading Video©

The speaker in the NTID Speech-Reading Video© (also known as the CID Sentences Video) is Diane Castle, Ph.D., audiologist and former NTID faculty member. It has been used as a test of speech-reading and auditory recognition at NTID since 1972. There are ten sets of ten sentences each. The purpose of this test is to assess the ability to receive information when:

a) Speech-reading alone
b) Speech-reading and listening combined.
c) Listening alone.

The sentences are from the CID Everyday Speech sentences developed by Central Institute for the Deaf in concert with the criteria set forth by the Committee on Hearing and Bio-Acoustics (CHABA) of the National Research Council. These criteria specified that sentences should closely resemble “everyday speech” in such parameters as vocabulary, sentence length, syntactical structure and redundancy.

The information gleaned from the use of this video will help the professional describe functional receptive communication skills and may be used to assist in planning aural rehabilitation.

LNT/MLNT©

The Lexical Neighborhood/Multisyllabic Lexical Neighborhood Tests (LNT-MLNT)© were developed primarily for use with cochlear implant children.  Determining the benefits of sensory aids (i.e. hearing aids or cochlear implants) is essential for the proper management of hearing impaired children.  Open-set word recognition tests have provided important diagnostic information relative to the benefits of sensory aids.  The information garnered from these tests indicates the extent of neural representation of words in a child’s long-term lexical memory.

The most used open-set word recognition test for children has been the PBK-50 lists.  However, it has been noted that for hearing-impaired children the PBK-50 does not assess the abilities of these children validly.  The LNT-MLNT© has been shown to be a more sensitive measure of spoken word recognition in these children.

The LNT-MLNT© was developed along theoretically motivated current models of spoken word recognition in listeners with hearing impairment.   The development of this test was based on the assumptions of the Neighborhood Activation Model (NAM), which proposes that words are organized into “similarity neighborhoods” based on their frequency of occurrence in the language and the number of phonemically similar words, or neighbors, within the lexical neighborhood.  A dense lexical neighborhood contains many phonemically similar words, whereas a sparse lexical neighborhood has few phonemically similar words.  Words that occur often and come from sparse lexical neighborhoods are easier for children to identify than words with the opposite lexical characteristics.

The stimulus words for this test were selected according to two criteria: 1. The words had to be familiar to young children with limited vocabularies. 2. The selection was based on current models (NAM) of word recognition and lexical access.  Stimuli on this test are separated into easy and hard word lists.

Time Compressed Sentence Test©

The Time Compressed Sentence Test© (TCST) is for identifying and quantifying disorders of auditory processing in children specifically the child’s ability to understand acoustically distorted or rapid rates of speech.  In addition, results of testing with the TCST may yield insights into children with slow reaction times and speed of mental processing. It is considered a monaural low redundancy speech test (monaural separation closure -MSC) sensitive to brainstem and cortical lesions. The developer states the TCST can be administered and scored in less than 15 minutes.

The Time Compressed Sentence Test© includes a) audio recording, and b) manual and score forms as printable pdf files. The extensive manual includes administration, scoring, and interpretation information. Age-specific normative data for ages 6, 7, 8, 9, and 10-11  is included. The developer recommends using the normative data for the 10-11 age category as a guideline for testing patients older than 11.

Pediatric Speech Intelligibility©

The Pediatric Speech Intelligibility Test© is a test for children ages 3 to 6. It fits in two important auditory processing domains. It can be categorized as both a dichotic test and a monaural low-redundancy speech test (monaural separation closure-MSC) sensitive to low brainstem.

The importance of speech audiometry, especially where the test materials include both monosyllabic words and sentences, has been well established for adults. Experience with performance vs. intensity (PI) functions and message to competition ratio (MCR) functions with adults led to the development of an analogous test for children; the Pediatric Speech Intelligibility (PSI) test.

The PSI includes both monosyllabic words and and two sets of sentences recorded in quiet and with competition (auditory figure ground). The test employs color plates with pictures of animals (animals were used to avoid ethnic biases) which represent either the sentences or the words. It is an extremely useful tool for the investigation of peripheral and auditory processing with children down to a language age of three or slightly younger.

Pitch Pattern Sequence – Adult Version©

The Pitch Pattern Test – Adult Version© (Standard) is an auditory processing disorder test for ages 9 and up using non-verbal stimuli. It is considered a temporal test using auditory pattern temporal ordering (APTO) and is sensitive to cortical lesions, interhemispheric transfer. It was developed by Dr. Marilyn Pinheiro.  Three tone bursts at either of two frequencies are presented monaurally.

The Spanish Pediatric Picture Identification Test-Male Talker (SPPIT-MALE) opposite Spanish Four Talker Noise© has the Spanish Pediatric Picture Identification Test (SPPIT) recorded to the left channel with the Spanish Four Talker Noise recorded to the right channel.  It is offered for those who want to administer this test with noise; however, no interpretative data is available for the test in noise.

This versatile recording can be administered for masking, speech in noise, or words in quiet. With a dual-channel audiometer, administrators can set their own signal-to-noise ratio by mixing the left and right channels. They can access the left channel only to administer the word lists in quiet. Administration time is 8-10 minutes.
