Speech Discrimination for Children

For speech discrimination testing in adults, audiologists often choose either the W-22 or NU-6. If they want to shorten the testing time, they will use a list that is ordered by difficulty or a short interval recording. However, the choice between word recognition tests becomes more complicated when it comes to children because there are several to choose from. I am often asked: Which test is best? There is not one clear answer because the best test depends on the age range of your patients.

If your patients’ age range is 12 and older, you can confidently administer the adult word lists such as the W-22 Form A or NU-6 Form A. Depending on the vocabulary of the child, you may choose to administer these lists to kids a little younger than 12. The administrator must start the recording, then the patient simply repeats the word he or she hears on the recording.

The PBK-50 Form A word list was designed for lower grade school age children. The PBK is similar to the W-22 or NU-6 word list except the vocabulary is more appropriate for this younger age group. Again, the patient simply repeats the word as he or she hears on the recording.

WIPI 2

The Word Intelligibility by Picture Identification (WIPI) fits a lower age range of 5 to 8 year olds. This test makes use of a picture book (sample at left). The child listens to the recording, then points to the pictured representation of the word as he or she hears it. There are six choices on each page. Aside from being easier for younger children, it is also beneficial for the test administrator to see the selection by using picture-pointing since the child’s spoken response may be difficult to understand.

image005The Northwestern University-Children’s Perception of Speech (NU-CHIPS) test reaches an even younger age range. Ages 2-5 can be tested by administering this picture-pointing test. As you can see from the pictures at right, there are less choices for the younger child to select from as they listen to the recording.

The test an audiologist administers to a child should be appropriate for his or her age group (or language age). If the test is too easy or too hard, the test results may not show his or her true ability to discriminate speech. Therefore, it may be necessary to purchase one or more tests for your patients depending on the patient age range. The W-22 Form A©, also known as the CID W-22, is a phonetically balanced test for word recognition (i.e. speech discrimination). They are made up of consonant-nucleus (CN), nucleus-consonant (NC) and consonant-nucleus-consonant (CNC) words. The talker has a General American dialect. The articulation function is approximately 4% /dB. This product includes all four lists of 50 words each (200 words total). The W-22 administration time is 10-18 minutes.

The W-22 Ordered by Difficulty© is a word recognition or speech discrimination test. It consists of four lists of 25 words each selected by D. Rose so that they are ordered from most likely to be missed to the least likely. If the first 10 items (or 9 of the 10) are heard correctly, the test is halted, since it is assumed that an individual with word recognition difficulty will be most likely to miss the most difficult items. Persons who miss more than one of the first ten items are given the entire 25-word list. The administration time will vary from 4-21 minutes depending on how well the patient performs.

*What is the difference between the WIPI First Edition and WIPI Second Edition?

  1. The original authors of the WIPI developed a second edition to clear up the ambiguity of the first edition. In some cases the picture contains a more modern depiction, such as the coke bottle is now 2-liter bottle. About one picture per page was changed.
  2. The word list was changed. Therefore, the WIPI recording must correspond to the correct version.
  3. If you are unsure whether you own the first or second edition, check the color of the cover. If you own one of the old printed WIPI First Edition books, the cover will be either blue or green. The WIPI Second Edition is a dark red/ maroon color.

Does Auditec offer a picture-pointing test for young children in other languages? Yes, a similar test is available in Spanish called the Spanish Pediatric Picture Identification Test. If your pediatric patient speaks a different language, consider employing a test with nonverbal stimuli such as the Sound Effects Recognition Test.

Nonsense Syllable Test

The Nonsense Syllable Test© (NST) (also known as the Edgerton-Danhauer Nonsense Syllable Test) is considered similar to a word recognition or speech discrimination test. Watch the video demonstration below. (Scroll down.)

Several advantages of nonsense syllable materials over words include: increased analytic accuracy, less contamination by memory effects, less susceptibility to word familiarity effects, easier test construction and more sensitivity to the presence of auditory pathology.

All the data collected on the NST have shown that it is as easy to administer and score these as most of the traditional tests. Score sheets are available in both the International Phonetic Alphabet (IPA) and an easy dictionary pronunciation format. The NST consists of two 25-element lists in six randomizations. It is considered suitable for both children and adults.

Normative data is included, but it is not age-specific because the test developer observed little or no age effects.

Everyday Speech©

Everyday Speech© is considered a word recognition or speech discrimination test. It consists of 10 sets of 10 sentences each with 50 “target” words in each set for word recognition assessment under contextual conditions. They can be employed in auditory training also. The sentences vary in length and are spoken with minimal inflection. Administration time can vary from 8-24 minutes depending on the number of sets administered to the patient.
