Geographic Inequality of APD Testing Availability

If you suspected a child was suffering from an auditory processing disorder, how far would their parents have to drive to get him or her tested? In certain states like New York, there is a possibility public school students could be tested right in their own school! Sadly, children in other states have to travel over 100 miles to receive APD testing!

If it is tempting to simply accept this geographic inequality of APD testing availability as “just the way it is,” consider the long-term effects of a society that does not offer necessary diagnosis and treatment. A parent writes, “It took my daughter until 13 years to be diagnosed and we began at age 4….APD isn’t well treated in our schools in California and it destroyed our family’s life. My daughter eventually ran away, started vaping and dropped out of school….APD destroys hearts. Every child deserves to be understood and to understand.” Do you agree?

Here are some practical ways to help:

  • Increase awareness of auditory processing disorder and ask parents to do the same. This could be in the form of writing blogs, public speaking, and reaching out to local special school districts.
  • Offer APD testing if you are a qualified professional. This could require reaching out in professional forums to ask for mentoring and advice.
  • Persuade health insurance companies to cover APD testing and encourage parents to do the same.

Nonverbal Testing

Researchers recommend hearing professionals use both verbal and nonverbal testing to evaluate patients. Nonverbal testing is especially useful when testing patients: 1) who do not use English as a first language, 2) who have language limitations due to hearing impairment, or 3) who use cochlear implants. Auditec offers a variety of nonverbal tests to suit the unique needs of hearing professionals around the world.

The Sound Effects Recognition Test (SERT) is a picture pointing test that uses sounds like a barking dog, whistling, or a vacuum. It includes instructions and normative data for ages 3-6 as well as the audio CD and the corresponding picture book.

Hearing professionals who are testing adults may find Environmental Sounds (also known as Forty Familiar Sounds) a beneficial test to add to their battery. This recording includes sounds like: bowling, roller coaster, and a babbling brook.  A similar recording called Filtered Environmental Sounds includes those sounds at specified frequencies. These recordings can be employed as the hearing professional sees fit. No normative data or instructions are included; however, a sounds list is included.

Nonverbal tests play a key role in auditory processing.  Auditec offers the Random Gap Detection Test, Gaps In Noise, Pitch Pattern Sequence, Duration Pattern Sequence, and Masking Level Difference-Tone Version to audiologists all over the world. The Auditory Continuous Performance Test© (ACPT) was developed to screen for auditory attention deficits. It can aid in the diagnosis of ADD or ADHD in children 6 through 11. MULTIPLE AUDITORY PROCESSING ASSESSMENT VERSION 2 (MAPA 2)©

The MAPA-2© is a comprehensive assessment of auditory processing and listening skills. It may be used as a screener to be followed by other behavioral or physiological tests or it may be used for a preliminary diagnosis in the auditory area. The test is administered via CD and can be used in a clinical setting or a sound booth. Administration time is 30-40 minutes.

The MAPA-2 includes eight different subtests in the three domains (monaural, temporal, and binaural) that ASHA and AAA recommend for auditory processing assessment along with the Scale of Auditory Behaviors, a 12-item parent or teacher completed questionnaire of listening behaviors.

Monaural: Evaluate an individual’s ability to use low redundancy information.

  1. Monaural-Selective Attention Test (MSAAT): Assesses an individual’s ability to recognize monosyllabic words with and without competing background noise of high-interest speech
  2. Speech in Noise for Children (SINCA): Assesses an individual’s ability to recognize monosyllabic word pairs presented with competing pink noise at progressively decreasing signal-to-noise ratios

    Temporal: Evaluate an individual’s ability to use acoustic information over time.

  3. Tap Test: Assesses an individual’s ability to recognize and label an auditory pattern
  4. Pitch Pattern Test: Assesses an individual’s ability to use cues including rate and pattern and to label what is heard

    Binaural: Evaluate an individual’s ability to use unique information presented simultaneously to each ear.

  5. Dichotic Digits: Assesses an individual’s ability to recognize digit sequences when digit pairs are presented simultaneously to each ear
  6. Competing Sentences: Assesses an individual’s ability to recognize more complex language sequences presented simultaneously to each ear

    Supplemental Subtests: Additional tests that can be used to assess an individual’s skill in the temporal area.

  7. Duration Pattern Test: Assesses an individual’s ability to use cues including rate and pattern and to label what is heard
  8. Gap Detection Test: Assesses an individual’s temporal resolution abilities using intervals of varying length between stimuli

Dichotic Digits – Standard©

The Dichotic Digits-Standard© test is an auditory processing disorder test using verbal stimuli. It is considered a dichotic test for binaural integration sensitive to brainstem, cortical, and corpus callosal lesions.

This established test assesses the ability to process different materials arriving at the ears simultaneously. Fifty sets of digits, one through nine (except seven, having two syllables) are presented either in single pairs or double pairs. The digits begin and end simultaneously. Administration time is approximately ten minutes.

Frequency Pattern©

The Frequency Pattern© is an auditory processing disorder test for ages 8 and up using non-verbal stimuli. It is considered a temporal test using auditory pattern temporal ordering (APTO) and is sensitive to cortical lesions, interhemispheric transfer. It was recorded by Dr. Frank Musiek.  Three tone bursts at either of two frequencies are presented monaurally. Administration time is approximately 10 minutes.
